Organizational Unit Overview Located in Greenville, NC, Internal Medicine is a department within the Division of Heath Sciences and Brody School of Medicine at East Carolina University. The Department of Internal Medicine is comprised of ten divisions with over 85 full time faculty and 150 support staff members. Each of the divisions has its own specialty clinic. Internal Medicine is home to seven fellowships and two residency programs. In recent years collaboration and funding have come from many national recognized organizations such as the National Institutes of Health, Health Resources and Service Administration, American Heart Association, NC Department of Human Health and Human Service. More locally, we have partnered with ECU Health Foundation , Kate B. Reynolds Charitable Trust , North Carolina Biotechnology Center , and the Pitt County Health Department . Additionally, Internal Medicine participates in many pharmaceutical sponsored clinical trials We collaborate with Department of Pediatrics , the Department of Surgery , and the Department of Cardiovascular Sciences within the Brody School of Medicine . The Department of Internal Medicine has a close affiliation with ECU Health Medical Center and serves a diverse population in eastern North Carolina.

Job tasks Nutritional assessment for this position includes but is not limited to conducting a diet history, collecting data from the patient regarding physical activity and other lifestyle factors including social determinants of health, the medical record, downloads from insulin pumps and continuous glucose monitoring systems, and evaluating the data. The process is necessary to develop a personalized care plan that include the patients' nutritional needs, previous intake, motivation, and capability to make lifestyle changes, obstacles to adherence and social support system. The Nutrition Specialist in this position provides initial and follow up medical nutrition therapy (MNT) focusing on diabetes and Diabetes Self-Management Education and Support (DSMES) for patients whose diabetes is complicated by -a number of co-morbid medical conditions including but not limited to obesity, renal disease, hyperlipidemia, cardiac disease, hypertension and other chronic medical problems impacted by nutritional status. Patients are referred from within the Department of Internal Medicine as well as Surgery, OB/GYN, family medicine and private physicians. The Nutrition Specialist must monitor and interpret subjective data, anthropometric measurements, laboratory results, and data reports from specialized technology related to management of diabetes to evaluate patient response to the prescribed care plan. Education is delivered in conformity with of the American Diabetes Association recognition guidelines. Family members are frequently included in instruction. Instruction is provided individually and in groups. The clinician in this position must be a Certified Diabetes Educator (or working toward this certification). The dietitian in this position also has a variety of tasks requiring skills in the area of adult diabetes management. Patients are comprehensively assessed for educational needs related to diabetes self-care deficits, medication adjustment, skills assessment, and physical assessments to include hypoglycemia or hyperglycemia, ketonuria and dehydration. Individualized teaching related to these issues is provided to the patient and available family members. This teaching focuses on building expertise of diabetes to include medications, metabolic mechanisms of diabetes, diabetes complications and their management, management of physiological abnormalities resulting from diabetes, and exercise issues. The teaching also builds skills, e.g., insulin and other injectable medication administration, blood glucose/urine ketone monitoring using glucose meters and continuous glucose monitors, and insulin pumps.
The Nutrition Specialist coordinates and manages all aspects of program planning and delivery for the Diabetes Self-Management Education and Support (DSMES) program which is a American Diabetes Association (ADA) recognized education program. Additionally, he/she acts as the Education Recognition Program (ERP) Quality Coordinator for DSMES. These tasks include: ensuring that program staff and educators meet the continuing education requirements and keep skills up to date in order for the program to maintain recognition which provides the capability bill and be reimbursed for delivery of diabetes education to patients. He/she is responsible for curriculum planning and evaluation, development of educational materials, and tracking and reporting of program data and quality metrics to the American Diabetes Association. The Nutrition Specialist co-coordinates and teaches in the ECU Diabetes Education Fellowship Program, a week-long program for physicians, nurses, dietitians and other health care professionals to develop skills in diabetes care and education. This program is offered 2 times a year in a group setting of 40-50 health care professionals. He/she coordinates participation of diabetes mellitus company vendors, manages the budget, responds to inquiries from potential participants and manages the registration of participants. He/she participates in planning for the ?Winning with Diabetes? program, a day-long education program for people with diabetes and their families. This involves superviseing the efforts of health care professionals from various organizations, and ensuring that all of the speakers, volunteers, staff and program materials are in place on the day of the program. The Nutrition Specialist participates in clinical research projects and other endeavors undertaken by the Section.
The Nutrition Specialist participates in educational conferences and seminars for the public and for other medical professionals. He/she is involved in instructing and supervising the work of dietic interns and other medical professionals. He/she should update and maintain working computer skills for word processing and electronic medical record use, maintain CDCES certification, keep updated in diabetes education techniques/nutrition issues by reading journals and related literature, attending diabetes regional and national meetings, networking with other diabetes educators and health care professional. The Nutrition Specialist in this position acts as a diabetes technology expert and function model for peers through the Diabetes Education Fellowship Program, local American Diabetes Association (ADA), and Association of Diabetes Care and Education Specialists (ADCES)) chapters, serves as a resource for area dietitians through phone contact and personal interactions, teaches diet instruction principles to nutrition, nursing and medical students, takes an active function in the local ADCES chapter by attending meetings and providing leadership abilities as budgets permit and teaches nutrition, diabetes education principles and other pertinent information at the diabetes Education Fellowship Programs.
After each patient encounter, the Nutrition Specialist documents the patient data in the medical record. Appropriate documentation is essential to receiving payment for patient services as well as maintenance DSMES recognized program status. SMART phrases and other SMART tools are used within the electronic medical record to document encounters according to guidelines for documenting the Nutrition Care Process (NCP) and DSMES. The Nutrition Specialist is responsible for keeping these SMART phrases and tools updated to meet industry standards for documentation of diabetes MNT and DSMES. The Nutrition Specialist is required to regularly share correspondence with the referring physician or others involved in patients' care.

Minimum Education/practice Bachelor's degree in foods and nutrition dietetics or public health nutrition; or completion of an ADA Dietetic internship with ADA Commission.
